filing the i-864 and i-765

I am almost finish filling out the forms. But for the i-864 what other documents would I need to send?

a) we both filed our 2013 taxes together since we been married for almost a yr.

b) 6 months of my wife pay stubs.

c) copy of birth certificate?

For the I-765 I just finished my OPT so I could just send copies of my i-20 and work permit card?

You need to send sponsor's employment letter showing salary and how long he or she been work, 6 months of pay stubs, tax return forms w-2, 1099, and 1040, 1040a or 1040 ez for previous 3 year.

I have a major issue and i dont want to mess this up.While filling the i -765 it asked what I was applying for? I had previously had the OPT and that had expire but since it would be a different type of card would it be a renewal of authorization or permission to accept employment?
