I was wondering, specifically what vaccinations are required for the 693? I do have my vaccination records & I would like to try & get whatever shots are required through my state health department as it is cheaper, then have my 693 signed by a civil surgeon.

I appreciate any constructive advice, Thanks in advance.

10/3/14- EAD 'Delivered'.. but never recieved :ranting:

10/16/14- Interview date received for 11/19 - My birthday! :D

11/19/14- Interview day- Approved- PR status registered & Welcome letter mailed

11/24/14- Welcome letter recieved

11/26/14- GC Mailed

11/29/14- GC Received

09/23/16- Mailed ROC package

10/03/16- Package sent back due to missing documents

If you are adjusting from Student/Tourist you need to do full I-693, which requires bloodwork, TB test, vaccination record.

All your vaccination record must be in English. If yes, then you can do as you intent above for vaccination assessment ONLY. Other parts still requires Civil Surgeon to perform for you.

I don't remember the exact list of vaccines off-hand, but it can easily be found online. And it is on the form as well. What I did was go to the student health center here to get all my vaccines and have that list looked over. They filled in the records for the vaccines I did not yet have (that they gave me), and then when I went to the civil surgeon, they just checked it all off, gave me a physical exam and a blood test (for syphilis) and that was that.

I also got a Quantiferon TB test (blood test) from the student health center. Much cheaper that way, and I preferred it to the skin test, and when I showed them the results they were fine with it and wrote it on the form.

06/06/2012 - My wife first comes to US on F-1 to study

05/19/2014 - We get married!

06/18/2014 - Mail in AOS forms.

06/20/2014 - Confirmed Delivered via USPS 2-day

06/25/2014 - NOA (I-130, I-485, I-131, I-765) via Email/Text Message

06/30/2014 - ASC Appt. Notice for Biometrics received!

07/01/2014 - Hardcopy of NOA received.

07/17/2014 - Appt. to take Biometrics

08/07/2014 - Ready for interview scheduling!

08/10/2014 - Received Interview date scheduled for September 16, 2014

08/27/2014 - Travel Document Approved!

08/28/2014 - EAD Approved!

7/30/14 (Day 40): Email update: I-485 has moved to "Testing and Interview" stage

8/20/13 (Day 61): Text notification: I-131 and I-765 approved, "Card Production" status

8/28/14 (Day 69): Text notification: EAD/AP card mailed

8/29/14 (Day 70): EAD/AP card received

Timeline

The doctor we had in New York only recommended TDP and MMR as the two mandatory ones. For Varicella and Hep, they do not require proof at least for me they didn't. My TB skin test was also positive [All Indian immigrants have BCG shots that contain the same pathogen minus the bad stuff] so a chest x-ray was mandatory which came out negative.

07.17.14 (Day 9) Re-sent package via Fedex 2-day priority with correct address

07.24.14 (Day 4) Received Text notifications and Receipt numbers at 2:30 AM EST for I-130, I-485, I-765, and I-131) - Lesson learned!
