You forgot your birth certificate, 6 passport-type pictures (2 for each application AOS/EAD/AP) and your check payment. Also, the I-765 and I-131 are processed separately from I-485; Thus, they have their own separate required documents. You will have to provide duplicates of some of those docs and paperclip it behind the corresponding forms, I-765 and I-131. Fill-out 3 G-1145 as well (one for each form).

http://www.uscis.gov/sites/default/files/files/form/i-131instr.pdf

By Tax return, did you mean the IRS tax transcript? If not and it's the 1040, you will have to submit the complete 1040 (all schedules) + W2 and/or 1099. These are supporting documents for I-864, so it will be a good idea to clip or place them behind the I-864 form.

Best to include a cover page to summarize all the files included in your application as well and do place the G-1145 on top of your forms. The VJ guide for AOS from K1 provides an excellent checklist: http://www.visajourney.com/content/k1k3aos

Edit: Btw, what are the court order docs for?

If using the 1040, you just have to make sure all the schedules are attached and you also need to submit the corresponding Wage and Tax statement W-2 and/or 1099. Most people simply use the IRS Tax transcript to avoid missing any page/s and which you can simply download online: http://www.irs.gov/Individuals/Get-Transcript

For the I-693, did you see a civil surgeon to transcribe your vaccination form? If not and your medical is within the year prior to filing AOS and all the required vaccines are given during the medical, you only need to submit the DS-3025.

My K1 AOS list. Try it as a checklist.

I-485 (Adjustment of Status)

Photographs (2) of immigrant (passport type)

Photocopy of birth certificate of immigrant with translation

Photocopy of passport biographical page and K1 visa page (immigrant)

Form G-325A Biographic Information Sheet (immigrant)

Form I-864 Affidavit of Support (by USC) with photocopies of supporting documents: This varies by each applicant so here's generic suggestions:

---Employment letter

---3 tax returns including copies of W2s (Photocopy) or IRS tax transcripts (photocopy) Only 1 year is required

---six recent pay stubs (Photocopy)

(If needed)Form I-864 Affidavit of Support (by Co-sponsor) with photocopies of supporting documents:

---Proof of co-sponsors citizenship/status, ie US birth certificate, bio page of passport, naturalization

papers, or LPR status

---Employment letter

---3 tax returns including copies of W2s (Photocopy) or IRS tax transcripts (photocopy) Only 1 year is required

---six recent pay stubs (Photocopy)

Photocopy of I-797 Notice of Action, I-129 Petition Approval (That's your NOA2)

Certified copy of Marriage Certificate (can be photocopy of certified marriage certificate)

Copy of Form I-94 Arrival/Departure record- (print it out at www.CBP.gov/I94)

Form I-693 with the immunization section filled out by a US Civil Surgeon, signed and sealed by him. Read the instructions for the form. OR photocopy of DS-3025 from UK medical if all shots were completed and recorded by Knightsbridge. One or the other, not both.

I-765 (Employment Authorization)

Photographs (2) of immigrant (passport type)

Photocopy of passport biographical page and K1 visa page (immigrant)

Photocopy of Form I-94 Arrival/Departure record

I-131 (Advance Parole travel document)

Photographs (2) of immigrant (passport type)CPB.gov/i94

Photocopy of passport biographical page and K1 visa page (immigrant)

Photocopy of Form I-94 Arrival/Departure record

Add a check for $1070 made out to the Department of Homeland Security paper-clipped to your cover page. Behind the cover page put a Form G-1145 if you want to be notified that they got your applications. The cover page will look similar to the above list telling what you have included in the envelope.

That's 3 separate applications with supporting evidence all mailed together in one envelope with one check, one cover page, and one G-1145. They will get processed in different places, so that's why the duplication of evidence. You don't have to do all three, but the second two are free if sent with the AOS and will get you

authorized to work or travel sooner than waiting for the green card to arrive in most cases. Green card could

take 6 months. Work and travel 60-90 days.

Do not work until the I-765 is approved and you have the EAD card. Do not leave the US until the I-131 is approved (K1s) and you get the travel documents or you will not be allowed back in. Once you have a green card, then no problem.

Note: The EAD and AP are now coming as a combo-card covering both things. AP used to be a paper document. and EAD a credit card size thing, as is the greencard (permanent resident card).

Read the instructions for the forms and guides on here, they tell you what to write on the back of photos. i can't remember but its there, it's something like name and / or alien number in pencil.

How I do attach my photographs to the paperwork and do they need any information on the back name, A N oetc?

I stapled the back top edge of a ziplock bag midway down a full sheet of computer paper. Dropped the photos in and zipped close. Added the page to the packet.

Let's say they dropped your photos on the floor. How could they be sure whose photos they were? Maybe the name and A-number penciled on the back would be a good plan.
