Proper format of nbi w/ aka. Help

So doesnt matter if incorrect given name comes first then surname then "y" then middle name?

"Everyone" has answered your question. But what I am wondering why are you asking, the form is self-explanatory and the 221g is straight forward about what name the embassy wants,

The form at NBI is very simple - fill in the blanks as they are labeled

That is fine.

d. NBI CLEARANCE (Green Form). Applicants aged 16 years and older must have a valid Record
Clearance (for travel abroad purposes) from the National Bureau of Investigation (NBI). Clearances should
be in the applicant’s current name, birth certificate name, maiden name and any aliases or nicknames used,
including different spellings you have used of those names. An official letter of explanation from the
NBI is required for any notation of “No criminal record” or "No pending case”. For immigration
purposes, an NBI clearance is valid for one (1) year from the date it was issued. The NBI website is

My 221g was delivered yesterday (june 25) and received by Manuel Manalaysay of Stanley Associates at ecoplaza chino roces ext. I called the immigrant unit today (june 26)and it says my documents are still not with them. What does this mean?
