Proper format of nbi w/ aka. Help

Mispelled name on bc: yula barbara

Firstname: eula barbara

Middlename: manalad

Surname: riomalos

I was asked for my 221g at usem to have nbi with aka.

They put on my aka: yula barbara riomalos y manalad.

Is the arrangement correct? And with "y"?

Pls help. Anyone who has similar case with me. Just wanna make sure this second time around

Yes, that's the correct format.

Im just being more paranoid this time.

Shouldnt be like this : riomalos eula barbara y manalad?

Or as what the 221g requires me to HAVE NBI WITH AKA FOR "YULA " BARBARA MANALAD RIOMALOS

Thats what the letter exactly said.

Do i need to tell the nbi to follow exactly what the arrangement in the 221g letter wrote for my aka?

The "y" or "nee" refers to the maiden name, so the format you posted is correct (yula barbara riomalos y manalad).

Another correct format could be LAST NAME, FIRST NAME Y MIDDLE NAME

I'm bothered with my nbi as well. I just noticed today that in my NSO BC my given name in upper area is Dianah that was in all my documents. When I look in the boxes in the bottom area it doesn't contain the letter H so it's only Diana. Will I need an nbi with aka as well?

They use Spanish on our name...

Girl.. You need aka too if there's any descrepancy on your name on your certified NSO bc..

So doesnt matter if incorrect given name comes first then surname then "y" then middle name?
