We got a RFE requesting an updated vaccination record. They want page 1 of form I-693 along with vaccination record attained from a Civil Surgeon put in a sealed envelope.

I called the nearest Civil Surgeon in our area and they asked me to Fax a copy of the Vaccination worksheet(which we have) and bloodwork(which we don't have). What is the blood work that they are asking for? They won't give us an appointment until they have it....

I guess you will have to ask from the physician's office where you first did your blood work to furnish you a copy. Then the US Civil Surgeon will transcribe the information to the I-693. In my case, the blood work was repeated all over again when we got an RFE for the I-693. But I guess it varies from one Civil Surgeon to another.

Did you explain it was for a K-1 and you had your medical already abroad? Let them know the USCIS has your medical documentation from your K-1 medical exam and you do not need another exam, just the vaccinations. If they still insist on the blood work, then find a different civil surgeon.

Did you explain it was for a K-1 and you had your medical already abroad? Let them know the USCIS has your medical documentation from your K-1 medical exam and you do not need another exam, just the vaccinations. If they still insist on the blood work, then find a different civil surgeon.

No I did not explain. Tried to call them back yesterday but they had already closed. Will call them Monday and see what they say. Thanks

Many of us have used the following and had no trouble with the RFE. As you can see by the evidence even in their own documentation it says a K1 does not need these additional steps that your RFE is asking of you. This should help your cause if you filed as a K1/K2.

You do not need the medical exam tests repeated. If your medical was done within a year of filing for AOS, that is all you need. No exam or parts/tests of it. You just need the vaccinations to be completed.

The updated vaccination record is needed if all the vaccinations were not completed prior to being issued the K-1. The civil surgeon should administer or verify the completion of the vaccinations and fill the form. There is no need for an entire medical exam again but being a free market and knowing you have to have their cooperation to get your green card many of the civil surgeons will not do just the vaccination form.

You should have copy of the Vaccination worksheet from your first medical exam (abroad). Show it to the Civil Surgeon's office and they should be able to tell you what needs to be done. In our case my husband only needed TB Skin Test and RPR (Syphilis Blood Test) which only took 1 day for the result and form completed.

Civil surgeons will tell you that you need things you do not really need. Some of them do it just to make money off of you, and some of them are just ignorant about the actual USCIS instructions and policies concerning the adjustment from special visa types like the K-1.

You were not required to get another TB or Syphilis test. Those are part of the medical exam, and someone adjusting from a K-1 is not required to get another exam, or parts of the exam again, if their K-1 medical was done within a year before filing for AOS. See all the links posted above.

Well I called Dr. Diane L. Rose in Metairie, La and she told me she would charge us anywhere between $200-$410!!!! I explained that we did not need a medical, only needed the vaccination records transcribed. She still insisted that it would cost minimum of $200...

Read a post here that reccommended Dr Horace Baltz in New Orleans so we will see him tomorrow.

This isn't a very interesting topic, but thought I would post so that others searching for civil surgeon + new orleans would come across it. I went to see Dr Horace Baltz at 3439 Prytania today to transcribe my vaccination record and he was happy to do so for FREE (although I paid $30 for this season's flu shot as an aside). Initially his nurse/receptionist (it's a two-man show at Dr Baltz's office) tried to get me to strip for a whole physical, but when I explained very clearly that i just needed him to complete the vaccination sheet with my updated records (i had the 2nd MMR at my own GP in London after the visa medical), he was willing to do so despite finding it "most unorthodox".

I don't think any of us really knew what we were doing to start with, but by reading the i-693 clearly and insisting that I did not need a full medical, we got there! I did not need the flu shot for the record, as I did already have one this year in the UK, but I know that the strains are different and although I may meet immigration requirements, I wouldn't actually have any extra protection against flu season, so I paid $30 cash for that, and they sealed up the forms, gave me a copy for my records, and I cycled home (not before grabbing a sandwich from Milk Bar round the corner from Dr Baltz's office!).

So, to clarify, I would recommend Dr Baltz to people in the metro New Orleans area looking for a civil surgeon for AOS- whether you need the whole medical done, or just vaccionation records transcribed. He may not have done it before, but hopefully he'll do it again after today!

Well we just got back home from Dr Baltz's office in New Orleans. No hassle no haggling. We were only in there maybe 20 minutes and were on our way with the vaccinations transcribed and page 1 of the I-693 form in a sealed envelope! He only charged $25!!!!! Highly reccommend Dr Horace Baltz
