Help with I-130 forms

For the I-485 form- which address do I write? (Canadian in US on visitors visa, married husbNd may 25th 2014 in US) please see attached photo of form

Also, what do I check for part 2- I am applying for an adjustment of status because: (see attached form) I should also note that I was told to send the I-130 and I-485 together that's why confused.

This may be a stupid question but when filling out the forms, do I capitalize everything?

Also, is it ok once I print it out to fill in some spaces with a black pen ie: I need to write "none" for the I-94 space.

One more thing: when it has the space (box) for ie: former wife/husband- do I write n/a, none or leave it blank?

I believe it would be H , Which is other basis of eligibility.

Everything is filed concurrently , Which means I-130, 485, G-325, I-765 , I-131 ( Yes there is no cost for EAD/AP with AOS package ).. You will also need the I-693 , Which is a medical that is required as part of the process as well.

I was thinking to check box "H" but someone said to check box "a" because it says something about getting an immediate visa number if approved.

If you're husband is a US citizen, check "A" on the I-485. A visa number is immediately available to you, because you are marrying a US citizen. All of the forms must be filed concurrently.

If I knew how to redact items, I would upload our forms as an example for you. I know this process can be confusing, but please check out the link to the VJ guide I posted earlier. They are extremely helpful.
