I came here on my K-1 visa filing for AOS. In I-485 instruction form it says:-

You only need to submitt a vaccination supplement not entire medical report.

I get my DS-3025 vaccination document worksheet. Do I need to just snd this with I-485 or some things else also?

In summary, if you have completed your medical within a year prior to AOS filing and that your DS-3025 is properly filled-out (with all the necessary vaccines given), you don't need to see a civil surgeon to have your vaccination worksheet transcribed on to an I-693. If this is the case, you only need to send in a copy of your DS-3025 with your I-485.

We didn't send in any new medical information when we filed for her AOS, but we had all her vaccinations current when we did the K-1. If you've updated them you should send that along with your application, if there is nothing new since your K-1 then you don't have to, although if you'll feel better, it won't hurt to make a copy and send it in along with everything else.
