Timeline

My wife did the medical exam for the K1 visa before arriving here, the sealed documents that were given to her were taken during the interview and were not given back. Where can I get this vaccination document?

Since it will not be possible (at least I don't think so) for the clinic in Mexico to send you a copy, you are going to have to see a civil surgeon to complete the I-793. Unless you have proof of the shots, he will be giving you new shots.

you do not need to see a CS for additional exam and completed I-693 if you are sure that your wife received required vaccinations at her medical examination, the completed D-3025 will be with the sealed documents and you can submit a note with the AOS package explaining this, but if her vaccination record is incomplete you will receive a RFE and yes, you will then have to see a civil surgeon.

Juarez does not give vacciations to K1s, you have to see a civil surgeon and get the vaccination part of i693 done

21 Aug 2013: I-129F Sent

11 Feb 2014: Visa APPROVED!

20 May 2014: Wedding!

The clinic in Juarez will NOT send them a copy of the DS-3025. They did not fill one out to begin with. Juarez does not give vaccinations to K-1 applicants.

OP > You need to find a civil surgeon that will not insist on doing a whole new medical and just do the vaccinations. You need the required vaccinations and an I-693, Page 1 and 5 (the vaccinations sheet). The required vaccinations are: MMR, Tdap/TD, varicella(unless you already had the chickenpox) and the flu shot(only during flu season, which it is not currently, so need for it at the moment).
