I came in US with a K-1 visa in May, and got married to my husband a week later my arrival. Now we are preparing to apply for my green card.

I have done my physical exam before i went for the interview in Guangzhou consulate. I did my physical in a designated hospital in Guangzhou and got my vaccination shots after my interview this April. I was given the Vaccination Documentation Worksheet (DS-3025) (Picture attached below )and a yellow book with my vaccination history and an sealed envelope says "To USCIS (Vaccination form)".

On my DS-3025 form, I had MMR, Varicella, and Influenza shots before i came to america. On the form, it ticked "insufficient time interval" for my MMR and Varicella shots, and ticked "Completed series" for the Influenza one. Td/Tdap vaccine ticked with"Not Routinely Available" and the rest all ticked with "Not Age Appropriate". I have all my information and the panel physician signed his name and date. For the results part, he ticked "Vaccine History Incomplete - Applicant may be elligible for blanket waiver(s) because vaccination(s) not medically appropriate (as indicated above).

I have read people saying if you have a "complete perfect ds-3025 form" then you dont need to find a civil surgeon for the I-693 supplement vaccination part. I'm just very unclear on the definition of a "complete perfect ds-3025" form.

Here are my questions

1. Based on my situation, do I have to get a civil surgeon and fill out the I-693 form? or can i just use my ds-3025 form(the sealed envelope addressing to uscis is a official copy of my ds-3025 form)

2. What does it mean by "not routinely available" do i need to get my Td/Tdap shot?

3. Because the MMR and Varicella vaccines have two doses. I only had one of each by far, do i need to get the second dose before applying for AOS? or I can wait until after i sent my application?

4. I also have copy of my X-ray results do i submit it with I-485 as well?

I'm allergic to Penicillin could that be the reason i didnt get the Td/Tdap vaccine?

Thank you! i've read this one, so i guess since i didnt get the Td/Tdap shot, its better i just go to a civil surgeon and get my vaccine done and fill out the I-693 form

Yes I think thats a safe bet. The fact that its marked as incomplete seems like you need a civil surgeon to complete the rest.

You only need one shot in a series for AOS. However, you did not get the Tdap/TD, so you need to find a civil surgeon to get that one and to transcribe the vaccinations onto an I-693. Do not know if they will suggest getting the next shots in the series for the others or not.

The x-ray is yours to keep, plus the USCIS does not accept digital media anyway(if you were speaking of the x-ray CD).

It's marked "Not routinely available" on your DS-3025, so I agree too that you need to go see a civil surgeon for the missing vaccine and have your vaccination record transcribed to an I-693. You can also have the Tdap vaccine done in your local Health Department as it's normally cheaper than in the CS' clinic, then just later bring proof that you had this plus your DS-3025 to the CS.

- You have to call around as the cost varies from CS to CS, and some might even insist that you to take the full medical which is unnecessary and pricey.

No need to submit the X-ray results with your AOS.

I just got my interview done and we do have that dilemna on the I-693 form..it was ask on our interview letter to bring it with us on the interview date but the officer didnt ask about it..during the process, we just send the DS 3025 cause that was the only thing that was ask for the I-485 application..my vaccination supplement was also incomplete with the eligible for blanket waiver but i do have my vaccination on TDAp but had a no check mark on Rotavirus..thats why we were searching for any answers here telling us to go to the CS but we follow our guts and went to the interview without doing anything on the I-693..we figured if they gonna ask about it, we just gonna do it after the interview and sent it to them..were not in a hurry anyway and a 1 or 2 weeks of delay is not that bad at all..i know every local office had different interpretation on it but it works on our local USCIS..hopefully we help you out! You will need to see a civil surgeon for the Tdap. Not routinely available means it wasn't given (they don't have that vaccine), but it is one of the required vaccinations for AOS. For the vaccinations you received that say insufficient time interval, since you received the first in a series, you are not required for AOS to get the remaining shots. AOS only requires at least the FIRST shot in a series.

Thank you for your advice!! I'm planning to get the second dose for my own health. yeah i meant my x-ray CD, I just mailed my AOS package last Friday, kept the CD with me ;) so I think I'm good. Thank you!
