Got my Passport with visa today. Will have CFO soon.i registered my self online.But seems no way i can add my Daughter who is 12 yrs old.Or i need to registered her separately? does she need to attend the CFO? is there any other docs or requirements i need for my Daughter?

Age 12 and under do not attend CFO, you just need to complete the registration form to get her sticker.

1. Photocopy of Immigrant Data Summary for USA-bound emigrant (must not be detached from visa packet)
2. Original and photocopy of DSWD travel clearance (click here for the new guidelines on issuance of travel clearance) i need this DSWD?

You are K-1, you don't have an Immigrant Data Summary sheet.

Nope don't need DSWD if your child is traveling with you.

G-325A is a U.S. government form not a CFO form, I have never read of CFO requesting it. Most everything asked for by CFO is in my link
