Timeline

Hey guys, i lost my Naturalization form and i need some help with form n-565. For part 4 it says complete if applying for a new document because of a name change, complete if applying to correct your document, or complete if applying for a special certificate of recognition. what do i do about htese questions, do i leave them blank, or should i write not applicable. It says complete IF any of the question ask pertain to you. I lost mine.

Please help, I'm lost

Timeline

Part 4 says Complete if iff applying for a new document because of a name change.

Since you are not, then don't do anything in part 4 leave un-checked. Your answer in part 2 explains why you are requesting a replacement certificate.

Part 6 does not apply either since you are not requesting a special certificate for a foreign government official, so don't do anything there either.

Again when you see "Complete if" in the section, you only need to fill in anything if that part applies.
