What name do you want on your Green Card and EAD? Have you used your married name before, like for SSN, drivers lic, or state ID? Use your married name if you wish.

As advised by Leatherneck, you can use whichever name you want to appear on your green card and use this on all of your AOS forms. For me, I used my married name, and even followed how it's traditionally done in Philippines; That is, I changed my middle name to my old last name. Some said that you may have to check first if this middle name change is acceptable in your state.
