Missing Birth Certificate Issue

My congress rep finally received a response from the embassy.

They informed her that they did not receive the birth certificate that was mailed along with the passport and police record. We also sent an email to them informing them the documents sent. They also stated that they sent an email to us...We did not receive this email.. So they have now solicited help from his country(Ghana)

We were asked to pick up his passport in January. In April, i emailed them but was informed that they are reviewing the documents and will let us know if they need additional documents.

So, i am very confused because everything does not make any sense.

Do embassies loose documents or are they looking for a different birthday certificate?

What is the best approach for this type of situation? Should i ask him to make an appointment to drop off the documents... i dont know if we can do sooo
