im getting my wife papers together to start her AOS , but im alittle lost my wife did her medical exam in domincan republic but she didnt do any vaccinations , she has all her vaccinations in record , do we still have to do a i693 or just do the ds 3025

Yes, the DS-3025 Vaccination record is sufficient as long as it's properly completed, and if the medical is done within a year of filing for AOS as well, there is no need for your wife to see a civil surgeon.

do i go to a civil surgeon to do the ds 3025 or can i do it myself ??

The DS-3025 is the vaccination record provided during/after the applicant's medical. Only the panel physician in her home country can fill-out a DS-3025 for your wife. she didnt receive a copy of it , but i think it could have been in her packet that she gave to US customs

There will definitely be a copy of her medical results/ records in the visa packet, but sometimes, they misplace this. If your wife is 100% certain that she has no missing vaccine from her medical and no medical issues needed to be addressed, you guys can simply make a note on the cover letter about this with details such as the date and place where she had her medical.

Hopefully, somebody from DR can chime in, because there are also reports here that some panel physicians from other countries don't routinely give out a DS-3025.

My fiancé received a copy of the DS-3025, but it also had some vaccinations that were not given due to not being available. We went to the health department to get the remaining immunizations after we got married. Then took the records to a Civil Surgeon who completed the I-693 that we submitted with our AOS.

Day 4    11-24-2017  NOA1 hardcopy arrived in the mail

Day 25  12-15-2017  Biometrics Appointment at Ft Smith, AR ASC

Day 104 03-02-2018 Notified Interview Letter Sent.

Day 151 04-18-2018 Interview at Fort Smith, AR 

Day 174 05-11-2018 Oath Ceremony at Fort Smith USCIS.

If she did not have vaccinations done at the K-1 medical and marked off on a properly filled out DS-3025, then she will need to see a civil surgeon for an I-693. She does not need a new medical, but she needs the missing vaccinations and page 1 and the vaccinations page of the I-693 filled out and signed by a civil surgeon. You can get the required vaccinations cheaper elsewhere, like the health dept., and then take the records of those vaccinations to a civil surgeon.

She will have to see a civil surgeon to complete the paperwork. Hopefully you will be lucky enough to find one who will not make you do the exam again (they are rare). My fiancee had to pay $300 to have the stinking exam here again for nothing because there were no civil surgeons willing just to give the vaccines and complete the paperwork.

11/28/14: Received letter (dated 11/20/14) stating that our case had been received at the Sacremento office and would be processed there and if we don't hear from them in 60 days to contact NVC.

12/9/14: Card is in production

12/16/14: 2 year card in hand!
