Motion to Re-open to Reconsider Decision for Denial. anyone?

Hi all, has anyone here filed for a motion to re-open to request reconsideration after an unfavorable decision?

We filed for my I-485 in August 22, 2013. On Feb. 13, 2014, we got an RFE for a medical exam (I-693), to be submitted on or before may 13, 2014. Did the medical exam, immunization with a US Civil Surgeon and sent the results in a sealed envelope. Received by USCIS 9based on FedEx notice of delivery on April 2, 2014. However, last May 23, we got notice dated May 15, 2014 stating that our application was denied because apparently my service file shows that I did not respond to the RFE (request for I-693). It looks like the document we sent has been "missing" or "misplaced" within the office if it appears they are not they are not in my file.

The letter also said that I could file a motion to re-open to request reconsideration of the decision for denial.

So -- has anyone here undergone a similar situation where you had to file for a motion? Any input? Advice? Will appreciate it.
