Medical, Asking for Report

So did my Medical with Dr cheema, and asked if I have had surgury which I have two on my hip asked me name of surgury didnt know how top of my head so googled and emailed them back with the info, the front desk person is asking for some report from my doctor

Anyone else had to get a 'report' from the doctor? for Surgery? my Family Dr shredded all my childhood records, but Hospital I can request takes 30days tho

How far back was the surgery done? I personally had surgery on my ear in kindergarten. Obviously I know what was done but I have no Dr that I can get a report from. I'm surprised they asked about it - perhaps it was the nature of your surgery that caused them to ask? Can your current family Dr comment on it or research at the local hospital if you know what was done?

PS - I can;t imagine it would be something that would cause you to fail the medical so I would not be too concerned

yeah maybe it was a mix up with front desk girls, as me and doctor couldn't remember what the official name of my operation was called, then I sent a Wikipedia email link to what I had, and maybe the girl thought I needed something official hopefully it will be cleared up, yeah my doctor i think can get the paperwork if need be just didn't know if this was normal as no packet said bring in prior surgery information

they want from my Doctor on his legal letterhead is Condition and Treatment for the report, simple enough I hope

Seems straight forward......you should have said you had body part replacements as part of the robocop program

Oh! I had to do this for my vaccinations.. I don't have a family doctor but, everything is in record with Health Canada, give them a call!!

Tell them it's urgent to fax the information over to Dr. Cheema and they should be able to do it.

The lady I sproke to and explained everything to was really kind and said she would get it done by the end of the day!
