Please can someone advise me as to what I should put for the following questions:-

Part 1 - Current USCIS Status.

I arrived on VWP which obviously has now expired so I am out of status - what should I put?

Expires on:

There was no date on my I-94 - should I just estimate 90 days from 1st November when I arrived?

Part 3

Non Immigrant Visa Number

Would this be the I-94 number?

Date Visa was Issued

I arrived on VWP - so should I put date of visa as the date I arrived in the USA or just put N/A.

I appreciate any help.

10/01/08 Biometrics completed

03/02/09 Case transferred to CSC

04/04/2009 Received approval letter

03/31/2009 Date of Decision

05/01/2009 Card production ordered

05/07/2009 Received card
