Hello everyone,

1- I have 5 names in my passport and all civil documents, and nothing mentioned about first or middle or last name, its just like this:

Name: ONE TWO THREE FOUR FIVE ( one id my first name, two my father`s and etc....)

so when my wife entered the dv2015, she ticked on NO LAST NAME,

2- on my wife`s passport it shows:

First Name: ONE

Last Name: TWO THREE FOUR

and this how she exactly entered

now we are filling the DS-260, I think there is no problem with my wife`s name. ( otherwise please advise)???

How can I enter my name in the two fields ???

your kind help is highly appreciated.

If You are called John Doe Lawrence Michael Smith Put John in ther name First The rest in the middle and Smith in the Last.

For your wife put Michael as the last name and her name as the First and the rest in the middle.

As long as you do not change the names as per your lottery entry you are good to go. =)

DO NOT MODIFY ANY NAMES.

Thank you sumranbi,

Actually in the DS-260 there is only two fields for names, Surnames and Given Name.

we selected as

( this how it shows when we opened the DS-260 for the first time.

my wife`s:

Surnames: TWO THREE FOUR

Given Names: ONE

my self:

Surnames: NLN ( No, Last, Name) As I entered

Given Name: ONE TWO THREE FOUR FIVE

so do we have to do some changes or no ???

This was my prob. I got 2 first names, middle name (maiden), last name. It says on the DS-260, enter the name as it appears on your passport so i put my first names and middle name on the first name field so i ended up having 3 first names on the first name field. When my ssc arrived, they got my name wrong and since they have limited characters on the first name, my middle name was missing 3 letters. Here are the problems i encountered because of that.

1. Wrong name on the social security card

- Corrected by going to local SSA office, they verified it through dhs and received my new card with the correct name after 10 days.

2. Name on e-verify was not corrected so lost job offers since the employers could not see me in the system. I just knew that they did not update my name when the job offer was withdrawn.

- Set an appointment with local USCIS next week to correct it.

My gc and ssn are correct. Called USCIS and SSN has the correct name but have to go to the local USCIS to check on the records why my name is still incorrect.

I suggest, once you get here, automatically call SSA once you receive your ssn with the incorrect name.

I kinda have the similar problem. When I got married Judge asked which one is my first name.

In my home country we use our father's name a surname. But we place it infront of my own name.

So my father's name looked like a first name as it appeared. The judge took my father's name as a first name and put my newly married husband's last name as a last name :)

Is USCIS going to deny my green card because I'm using my father's name as my first name?
