Are the costs standard charge of $450 ? for the required Medical or does this vary from State to State.

There is indeed a wide variety in fees charged from one Civil Surgeon to another. If you can, it pays to shop around.

The USCIS does not set prices or regulate what the civil surgeons can charge. You just need to call around to find the one that charges the least amount.

Mine was $220 total. Physical was $150, $50 for the blood test/TB skin test and then $20 for the flu shot I needed. Had my TB skin test been positive or if I needed more vaccinations, would have been more.

I'm in Alabama though, stuff is much cheaper here. I called around to 15 different CS's though to find the best deal.

We did not go a CS and submitted the DS 3025 along with these instructions: Green Card in hand with no RFE!

3. What if I am a K nonimmigrant visa holder and already had a medical exam overseas?

If you were admitted as a:

a. K-1 fiancé(e) or a K-2 child of a K-1 fiancé(e); or as a

b. K-3 spouse of a U.S. citizen or a K-4 child of a K-3 spouse of a U.S. citizen; and

c. You received a medical examination prior to admission, then:

(1) You are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination, and the medical examination did not reveal a Class A medical condition, or if you did have a Class A medical condition, you received a waiver of inadmissibility and have complied with the terms and conditions on the waiver.

(2) Even if a new medical examination is not required, you must still show proof that you complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original overseas medical examination report, you will have to have the vaccination report completed by a designated civil surgeon. In this case, you are required to submit the vaccination record and page 1 of Form I-693.

Paid $200 in Atlanta, ga. The place was packed solid full of people, two hours I was in and out, it was cheaper than most places annnnd probably why it was so busy?

It varies. I called all the CS in my area (within a reasonable drive) and found prices as low as $135 up to $400 - and this was for what I needed: I have all vaccine records as well as a current TB test (through work) and STD test (recently pregnant - so the MD accepted those lab results too). So I didn't pay for vaccines, X-Ray, TB test or bloodwork.

I'd suggest you go to the list of CS in your area and call around.
