CFO tomorrow. Does anyone knows where the forms at?

I'm going to get my CFO tomorrow. I just print out the schedule that I made online with my photo and barcode. Some applicants says that there is another form that I need to fill up. I wonder if I can find it in the website, if yes, I would love to know the link. Is there any other applicants that will attend CFO Seminar tomorrow? Please send your Viber account or PM me. I am chatting with Candy now. She just had her CFO today. Definitely bring his tax return (SS# blotted out). They asked for the one I supplied Candy with.

Also bring photos of the two of you together. They asked for that.

And two photo IDs. Defintely need that too. They accepted Candy's ID from Hong Kong, even though it is foreign. We were not sure if they would accept that so she brought an NBI as a backup. DId not need that.

She would have been in trouble without these things. Make sure you have them.

I think you are referring to this form: http://www.cfo.gov.ph/pdf/downloadable%20forms/registrationform.pdf

If you haven't had your visa yet, they won't ask you to fill this one up. There is another form that you will fill-out personally at CFO.

Just a little uncertain though if this procedure has changed since the online system implementation.

If you haven't had your visa yet, they won't ask you to fill this one up. There is another form that you will fill-out personally at CFO.

You are correct. I prepared the online form and emailed it to Candy, and that is exactly what happened. Still if the OP is pre-visa it would be useful for her to prepare it anyway to make filling out their form easy. It would just be a matter of transcribing the information to the other one.

They are quite serious about the phone numbers. They asked Candy for her mother's phone number and my phone number and told her they plan on calling both of us.

Since Candy has had 4 OFW posts already, she is quite familiar with how to protect her interests while living in a foreign nation. But I suppose an applicant without foreign experience could benefit from the knowledge at this seminar.

They asked Candy about living arrangements, plans to have a family, and plans to work in the US. Her answers (all truthful of course) were very generic. I own a single family house where we will live, both of us are too old to start a family (57 and 41), and she does intend to work full time here.

The OP should not expect any problem, and if they teach her something she did not already know, that is a good thing.

Do you think I need to print a copy of this and have it on hand before I proceed to the seminar or is their any form like this available in the CFO building?

Yes you need a copy of the visa and of the bio page of your passport.

They have the form in the building, but if you do it online that will save both you and them some time and get you out of there sooner.

What about this form that sweet shared to me. Do I need to have it on hand as well? With Bisa and Passport Bio, how many copies I need to make?

Yes. When I had my CFO seminar, they took this form from me. But at that time, it's only required for those who had their visa. Might as well have 2 copies of each: CFO form, visa and passport bio. If you miss a copy, there's a photocopy station outside CFO, but a little pricey I heard.

p.s. There will be another form that you need to fill-out by hand at CFO, different from this online form, but as far as I can remember, it asks for similar information. :)

Have you checked out Hank's guide yet? CFO's list is here: http://www.cfo.gov.ph/index.php?option=com_content&view=article&id=1348:fiancee-spouses-and-other-partners-of-foreign-nationals&catid=140

  1. Two (2) valid identification cards (IDs) with photograph;
  2. Duly completed guidance and counseling form;
  3. If married, original and photocopy of marriage contract on security paper from the National Statistics Office, or Local Civil Registry Offices; or original and photocopy of marriage contract duly authenticated by the Philippine Embassy/ Consulate (if married abroad)
  4. Other documents as may be required by the counselors; and
  5. Payment of P400.00 counseling fee.

#4 Requirement is quite vague. They seem to have added "more" required docs which vary in each applicant's case. This includes petitioner's BC, passport bio page, 1040/ IRS transcript, divorce decree if applicable, and some pictures together.
