DS-2035 & I-693 - Acceptable??

Timeline

Hello everyone.

I wasn't sure if this was the right place to post this but I've seen members of the UK boards give great advice regarding this, so thought I would post here first.

Anyway, here I am in the US, getting all my AOS documents ready. At the moment I've included photocopies of my DS-2035 in the AOS package but I'm wondering whether to get it transcribed onto a I-693 instead. From what I've read, my DS-2035 looks like they would accept that version (boxes checked, incomplete, etc) but was just hoping someone could take a look at my form and see if its acceptable to send it in as it is?

So should I send this in or get an I-693 done?

It's perfect. Use it.

The nurse doesn't need to write those "due in _____" but she does. It's not in the technical instructions to note that. But it's still okay.

Mind you, USCIS can fail to match up your medical file with your AOS file. They do that more than they should. If that happens you can get a RFE to do the whole medical over. Nothing you can do about that. And if you have an interview, any given adjudicator might not have studied their handbook and could tell you anything. So while there are no guarantees, hundreds have been approved with what you have.

Thank you so much. You're so helpful all the time :) Knowing my luck they probably will lose my medical file but in the meantime I'm just going to send it in like this. Thanks for taking a weight off my shoulders :)
