i-693 Phone in/Fax a Civil Surgeon

Timeline

I am just getting my head around filing, and have started the process of filling out the forms needed. I am a little perplexed as to the need of a i693 form, but I suppose I have to get it filled in, although I don't understand why a DS 3025 shouldn't suffice, as they force you to use their specified doctors. Anyway, I digress, and rant a little...

My question is, am I able to get the portion required to be filled in (Am I right in thinking only the Vaccination Supplement?) by a civil surgeon through fax or phone? I am having a hard time finding a Civil Surgeon, which is still active that would do it over fax.

I am not sure I need it, I am just reading around the forums, and everyone seems to be saying I need to have a civil surgeon fill in an i693, regardless on the completed-ness of the DS 3025.

For the record, I entered on a K1 visa.

The DS 3025 has been fully completed. I am just confused. Help is greatly appreciated.

K1 AOS:

May 27th: Notification via text/email confirming receiving package. Day 5

May 29th: NOA's recieved on 29th May Day 7

June 2nd: Biometrics appointment for June 16th arrives. Day 11

I was in the same situation as you and just as confused. I went ahead and (probably unnecessarily) got the I-693.

I understood that if your ds3025 says "complete" then you dont need a civil surgeon just a copy of the ds3025.

I understood that if your ds3025 says "complete" then you dont need a civil surgeon just a copy of the ds3025.

His visa medical was in London. They don't mark the DS-3025 "complete". The Technical Instructions for Panel Physicians says most will be marked "incomplete" and "may be eligible for a blanket waiver" because many shots are not age appropriate for adults. If the needed shots are documented, and waivers marked for the rest, then his DS-3025 is acceptable if the results say "incomplete" and "may be eligible for a blanket waiver" . Edited by Nich-Nick

Janet&adonis, my ds 3025 marked as complete but I went ahead and see a civil surgeon and guess what? I need TDap shot and had my I-693 updated.

Hey guys, sorry for posting the same question but I am still a bit confused even after going through all the links provided above. I just did my timeline too before posting this but I dont know if it's visible yet. Anyway, here's my story:

I came here on a K1 visa from Romania, on the 7th of March, got married to my American fiance on the 26th of March and now we're trying to put everything together to apply for AOS. My biggest confusion right now is about the damn medical exam and I-693. I had a full medical exam in Romania, as required, prior to having my visa interview.They sealed the results of that exam in an enveloped and told me not to open it until I arrive in the US where I am to hand that sealed enveloped to the officers at the point of entry. I did just that and now I'm left with nothing to attest that I had that medical exam EXCEPT the copy of the vaccination record (DS-3025). On this DS-3025 , the results section says something like this:

"Applicant does not meet vaccination requirements for one or more vaccines and no waiver is requested."

I looked over the list of vaccines on my record and they are all marked with either "complete" or "not age appropriate" WITHT THE EXCEPTION of the TD/TDAP which is marked as "insufficient time interval".

So, what I did is this: I contacted the nearest Civil Surgeon to the place where I live and got an appointment for this morning so that he can take a look over the vaccination record and tell me what vaccine I'm missing, if any. He established that I indeed needed a TD/TDAP shot and they gave me the shot. Now, here's the confusing part. When I asked him to fill out form I-693 for me, he said that he cant do that without knowing the results of my medical exam overseas...I told him that he probably doesnt need to fill out the entire form but just the vaccination chart plus page 1 but even so he said something about me needing a syphillis and tuberculosis test and that he cannot fill out Summary of Medical Examination section on the first page of the I-693 because he doesnt have any way of knowing what were the results of my medical exam.. I m kinda starting to think that he either didnt have a clue of what he was supposed to be doing or he was just trying to bullshit me in order to get some money. Because as long as I had that medical exam done before my visa interview at the consulate in my country, in Feb 2014 (just one month before coming here), it's impossble not to have had all those required tests already run on me.

Ultimately, he said that I should call USCIS and ask them to send me a copy of my medical exam or something and then go back to him...:(

What do you guys think? What should I do? Should I go back to him and have him complete my vaccination chart? Will that alone be sufficient or is it ABSOLUTELY necessary that I also have the first page of the I-693 filled?

Your DS-3025 was not filled out properly in the results section so you do need a civil surgeon.

"Applicant does not meet vaccination requirements for one or more vaccines and no waiver is requested" will not fly.

You need to read the instructions for the I-693 and get the civil surgeon to read them also. Page 5

2. What if I am a K nonimmigrant visa holder and already had a medical exam overseas?

If you were admitted as a:

A. K-1 fiancé(e) or a K-2 child of a K-1 fiancé(e); or as a

B. K-3 spouse of a U.S. citizen or a K-4 child of a K-3 spouse of a U.S. citizen; and

C. You received a medical examination prior to admission, then:

1. You are not required to have another medical examination as long as your Form I-485, Application to Register Permanent Residence or Adjust Status, is filed within one year of your overseas medical examination, and the medical examination did not reveal a Class A medical condition, or if you did have a Class A medical condition, you received a waiver of inadmissibility and have complied with the terms and conditions on the waiver.

2. Even if a new medical examination is not required, you must still show proof that you complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original overseas medical examination report, you will have to have the vaccination report completed by a designated civil surgeon. In this case, you are required to submit the vaccination record and page 1 of Form I-693.

You aren't going to get a copy of the medical results. Not appropriate.

You do not need a new exam. USCIS says so in C. 1.

The CS does not have to fill out medical results. USCIS says so in C. 2.

Yes you do page 1. USCIS says so in C. 2.

Thank you for the info. However, I'm afraid that things are still not fully clear yet regarding the first page of form I-693. So, on that first page we have the following:

Part 1. Information about you (I understand that this is something that I'm going to fill in, no problem)

Part 2. Summary of Medical Examination (to be completed by the civil surgeon). Now, this is what I dont get. Does he have to fill this part 2 out or not? Because I asked him today and he said that he cannot do that because he doesnt have any way of knowing what the results of my medical exam overseas were (more specifically he doesnt know what to check from the "NO class A or class B conditions, Class B Condition, Class A condition. I'm positive that I am a No class A or B conditions because I've never had any serious health problems but I guess he wont take my word for it..)

Part 3. Civil Surgeon's CErtification. Does he only need to provide his signature and info here without filling out Part 2 or how does it work?

I'm sorry for insisting but I m really confused and need to know exactly what I have to do.

This is about the 3rd or 4th version of the I-693 I have known. When this form came out, we discussed it was going to be confusing to civil surgeons and make the I-693 for a K1 even more of a problem. I know exactly what the civil surgeon means. It is like he is signing off on a full medical he didn't do. And they left off the signature area of the vaccination sheet. It is very poorly designed for a K1.

I do not have any examples of what other Civil surgeons do. My personal opinion is if he drew a diagonal line through Part 2 which refers to the full medical, it would still be valid. And he might agree to sign.

By signing part 3 he is basically saying

--he is a civil surgeon

--he has a license to practice medicine in his state

--he verified your identity

--he performed the examination according to the Technical Instructions (he did follow the immunization instructions)

I don't see any reason that would not be acceptable to USCIS. And that takes him off the hook for providing a summary conclusion to a medical exam he did not perform.

Or he could write in Part 2 "N/A. See K1 medical exam performed in Romania on [date]"

Maybe one of those suggestions would make him feel less liable for signing something he didn't do. I totally see his reluctance.
