DS-160 passport issued country & medical check questions

Hello,

We are in the process of filling out the DS-160 application for our K1 visa.

We have 2 questions:

  1. My fiancee (the beneficiary) passport was originally made in Indonesia (she's an Indonesian citizen with a Singapore PR), and the last issued place is the Indonesian Embassy in Singapore. Do we fill out Indonesia as the country that issued the passport and fill out Singapore as where the passport was issued? See attachment: post-177712-0-89685600-1400197166_thumb.png
  2. She has not done her medical checkup yet. DS 160 asks if she has immunization according to the US standard. Is it ok to say 'No', and explain that she'll do her medical checkup later? Or is it better to wait till she's done with her medical checkup and then submit the DS160?

1. Does the County of Issuance on her current passport say Singapore? Then she should write Singapore.

2. The immunization question doesn't really affect the medical check up. I had gotten immunization according to US standards because of a prior F1 visa (so I ticked yes), but I unfortunately lost all my records. During the medical check ups, they will ask for her vaccination/immunization records. It's okay to say no. It's just a question and doesn't really affect the medical check up.

Aug 1, 2014 - AOS/EAD/AP package delivered to Chicago Lockbox

Aug 6, 2014 - NOA1

Aug 19, 2014 - Biometrics appointment for AOS & EAD

Sept 29, 2014 - EAD/AP combo card received in mail.

Mid-Nov - Potential Waiver of Interview letter received

End of May 2015 - renewed my EAD/AP, got the NOA1 and another letter stating that I don't need to do biometrics appt.

June 11, 2015 - welcome to America letter! (no interview...no service request..)

June 16, 2015 - green card in hand

1. Does the County of Issuance on her current passport say Singapore? Then she should write Singapore.

I just answered yes to that question because I technically did when I got my student visa years back.

But my friend answered "no" to that question and it was fine. She just had to get the shots when she had her medical.

i went to tan tock seng travel health clinic and they were extremely professional and fuss-free.

you should put indonesia, not singapore, as the country of issuance. singapore cannot issue you an indonesian passport.

He should put Singapore as the country of issuance, since the passport was issued by the Indonesian consulate in Singapore.

Many years ago, I had a Singapore passport issued in KL, and so I had to fill in Malaysia as the country of issuance in official forms. Hope that helps.

country/authority that issued the passport = indonesia. the DS-160 guide says that this is the same as the applicant's country of citizenship.

place the passport was issued = singapore.

they are two separate questions on the DS-160 (see OP's attachment). the country of issuance is still indonesia. i was correcting the terminology.

Flying to Seattle on 6 May 2014!
