Pre-marriage and post-marriage surname issue (I-693 related)

Hello,

Today my wife, who now carries my last name had the Measles-Mumps-Rubella shot done. Now we are ready to get a civil surgeon here in NYC to transcribe the records for the I-693 form. The lady administering the vaccine today asked my wife for her name, and then asked what name she would be applying for the green card under. Obviously she will be applying under her new name.

The issue is that this latest vaccine she got done is under her new name, and all of the rest of the physical that she go done in Kobe, Japan is under her maiden name. Will this be an issue?

Not at all. Her marriage cert explains the difference very clearly.
