I have not seen Candy's DS-3025 yet since she does not have convenient access to a scanner, but I suspect that one can tell from reading it that she needs the 30 day booster for the MMR. The doctor told her expressly that she will need it, and my research on the net seems to indicate that this is the norm.

Once she gets it, whether in St. Luke's or with a US doctor here, how does that fact get documented? Do I have to go now to a CS to get the I-693 just for this one shot?

I don't mind going to CS if I have to, but if I can avoid it with smart planning now, I would rather do the smart planning.

You need only send a copy of the DS-3025 as is.

If she skirted out on the required MMR booster, would they know the difference? I want her to have it anyway because adults can ger seriously sick from these diseases. But you mean I don't have to document that she got it?

The DS-3025 is all that is required for filing AOS normally - with certain medicals being the exception.

Only the first in the series of shots are required for AOS. Did you see Nich-Nick's explanation that Stevephoto linked? Specifically:

Do you need an I-693 immunization sheet? Is my DS-3025 filled out properly?

Look at your DS-3025. If it has the ALL of the following items, you don't need an I-693 (Adults 19-49 yrs old).

1. Shots marked with a date (at least one of a series)

MMR (two are required but if you have "insufficient time interval" checked, then you get a waiver)

Td or Tdap or DT or DTP or DtaP (One no longer than 10 years ago, ie.have a booster)

Varicella or VH written by it if you had chickenpox

Influenza (during flu season)** see note below because you are most likely okay without it.

2. Everything else marked not age appropriate, insufficient time interval, not flu season...things that allow a waiver.

3. RESULTS section filled in with "incomplete" and "may be eligible for blanket waiver" ticked or "complete" (but complete is rare).

4. Signed and dated by the doctor

If it's not completed EXACTLY like that then see a civil surgeon for the I-693. If it is complete, send a photocopy of your DS-3025. Almost isn't good enough. It needs to be filled out properly.

Somebody said he got RFEd for not having the booster. Was he wrong?

I asked Candy for a scan of the DS-3025, and then I will analyze it.

You can order the NSO birth certificate of your fiancée online. That's what I did with mine - very hassle free. You can pay via credit card or bank deposits.

I concluded that myself and told Candy that if she can get it while she is in Manila without a big hassle to please do it; if not don't worry we will get it when she is here exactly that way.

As to the MMR booster, I want her to have it anyway because those illnesses are very serious when adults get them. But if there is some way USCIS expects me to document it I want to do that up front instead of waiitng for an RFE.
