Medical Report From St. Luke's GIven On CD

First of all I am happy to report on Mission Accomplished. Candy passed her medical and got her shots. They told her to get one booster in 30 days, which St. Luke's will give for free if she is still there; if not I will pay for it here.

Candy reported that the whole process went smoothly. There was a lot of waiting time, which should not surprise anyone and is good practice for the interview anyway. But the nurses and doctors and techs there were professional and competent, and there was a good system to keep a regular stream of applicants going to all the medical people, making efficient use of their time. Her only complaint was that the tech assigned to draw her blood was a little heavy handed with the needle.

She got on line at 4:30 AM and got out by 11 AM, as it pays to get there early. The next day she was assigned 7:30 AM for her shots and was held there until 2 PM, about 95% of if just waiting (big surprise).

My question is what they gave her on the way out. She got the vaccine report, which I expected and which I know we will need for AOS, but there were no other papers records. The report was issued on CD, in a sealed envelope, and she was told to present it to the immigration officers at POE. St. Luke's said they will send to the embassy whatever the embassy needs and there is no need for her to bring anything medical there.

This is contrary to what I have read and seems to be something new. Has anyone who got a recent med exam experienced this? I don't at all mind them doing it this way; in fact I am glad to see this 21st Century way of doing things instead of using 20th Century paper. I was just wondering when this started and who else has had theirs done this way.

The CD is the copy of the x-ray. The medical exam result will be handed to the US Embassy. So she won't have any copy and that is for the interview at the Embassy. I am not sure with the AOS medical requirement tho.

You just need the vaccination report when you do your AOS after she gets in the US and you get married within 90 days.

You would need to find a civil surgeon to transcribe the vaccination report to another form (no need to do another medical); it will cost you less than 50 dollars for this official transcription. The civil surgeon will hand you a sealed envelope, which you will then send along with the other requirements for Adjustment of Status (I-485). As for the CD, I haven't had any need for it, so far.

So far, so good. :) I hope she doesn't forget about the need to attend the CFO seminar and get the sticker on her passport.

Note: I did my St Luke's medical exam Sept 2013. I am now in the US, married, waiting for my Permanent Residency card after sending the AOS papers last Dec. 30.

Nope, nothing new regarding medical file being sent to the embassy... that is the way it always is, SLEC sends the medical file to USEM, eventually it will be inside the DO NOT OPEN ENVELOPE.

The CD is just X-rays, they don't need it at POE either, the DS-3025 vaccination record is important (copy is needed for AOS), have Candy keep it with her personal documents she is bringing to the USA. Only the DO NOT OPEN ENVELOPE needs to be handed over to the immigration officer at POE. It would also be good to have two extra copies of the NSO birth certificate to bring to the USA.

Hank, so the CD is not needed at all? It has a seal on it, so it seemed the seal was there for a reason.

As to the 2 new NSOs, do they need to be certified and on security paper like the one she has ready to hand to the embassy? She has 4 business days of idle time now in Manila before her interview, so I will send her to get those now if we need them.

Nope not needed at all. If you take a poll you will find thousands that still have that CD sitting somewhere, or they tossed it. My wife has hers in a folder somewhere, we will use it for target practice one day ;)

Yes request two more birth certificates from NSO, you will need an original for your own use and the Philippine consulate requires one when you do ROM.

She has the second NSO for the CFO; we were prepared for that. What is the third one for?

Also I am pretty sure I can have my own doctor do the booster shot. If we go that route, what do I have to get from him to show the civil surgeon it was done?

I am not talking about CFO I was mentioning the Philippine consulate here in the States for Report of Marriage later, they want an original of the BC

CFO will only need to keep a copy of the NSO birth certificate.

But if she gets her visa fast I am not going to hold the plane up for a week or two just to save the cost of one single shot. If I use a doctor other than the civil surgeon to give the shot, what do I get from him to show the civil surgeon it was done?

c. You received a medical examination prior to admission, then:

(1) You are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination, and the medical examination did not reveal a Class A medical condition, or if you did have a Class A medical condition, you received a waiver of inadmissibility and have complied with the terms and conditions on the waiver.

(2) Even if a new medical examination is not required, you must still show proof that you complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original overseas medical examination report, you will have to have the vaccination report completed by a designated civil surgeon. In this case, you are required to submit the vaccination record and page 1 of Form I-693.

No, we just sent in the 3025 (and a copy of the instructions I posted above). It was only mentioned in passing at the AOS interview. Joan did not need any booster. We did show the vaccine form to our personal physician and he said that she was up to date and good for a long time.

[Edit] Where are you from in New York? I am from Long Island originally. I took Joan "home" to meet mom over Christmas. It was her first time to see real snow!

You only need a copy of the DS-3025 (keep the original) unless you fiancee had a special medical condition (per email conversation I had with USCIS). Some adjudicators at USCIS don't seem to know this and will RFE for the I-693 without reason. I have suggested to others to staple a copy of page 4 of the I-485 instructions with section 7c highlighted - help educate the adjudicator. To date no one has had an issue sending in the DS-3025 that has followed this suggestion.

I have not seen Candy's DS-3025 yet since she does not have convenient access to a scanner, but I suspect that one can tell from reading it that she needs the 30 day booster for the MMR. The doctor told her expressly that she will need it, and my research on the net seems to indicate that this is the norm.

Once she gets it, whether in St. Luke's or with a US doctor here, how does that fact get documented? Do I have to go now to a CS to get the I-693 just for this one shot?

I don't mind going to CS if I have to, but if I can avoid it with smart planning now, I would rather do the smart planning.
