Package 3

Hi all

I just received the package 3 and started filling it out

I'm at question 14 where it asks name of spouse and birthdate along with marriage date

My wife passed away 2 yrs ago how do I answer this question I already marked in the box on question 9 widowed

Also question 19 asks to list all my children They're all grown now and have families of thier own and won't be accompanying me to the U.S. Do I still list them.

One other thing on the check list they want, some items don't apply like court prison records or military record do I just put NA in the box

Is there a fax # I can fax the checklist back to them at ?

Another one question 22: in what occupation do you intend to work in the United States what will I put in there I don't havea job lined up as of yet
