There are a couple of reasons. The main one is the interviewer was not 100% satisfied that the petition warranted a visa at that time and wanted MORE info than initially asked for.

The best way to avoid this is to show a strong evidence initially and do things that show a strong relationship. make sure that the one being interviewed is well prepared.

But why was OP's fiance given a blue slip for missing those forms? I thought G-325a forms submitted with the petition would already provide the information of residences in last 5 years.

Thanks guys. I'm about to send my fiancee the documents so I think to be safe I might as well include this form with the package to save any hassle in the future.
