Question about reading translated Vaccination Page

So, I think what I'm going to do is take my wife to the city's Free Clinic in order to get her "Measles-Mump-Rubella" vaccine. She already has all of her other vaccines according to the medical she did last October in Kobe, Japan. I went by myself to the clinic today to ask them if it would be fine to just bring her in to get the vaccine, and they couldn't really read the paper(it is in English however). I can barely read it myself. They were trying to tell her that she needed the Polio vaccine, but I doubt that.

I think I'll have her get the MMR vaccine, then I'll find a civil surgeon that won't try to lie to us and will help us with our form, then I'll send it in with the other AOS paperwork, and have them send an RFE in case there is something missing.

Does this sound about, right? I'm almost tempted to upload a picture of my wife's vaccination form to see if you guys that have experience reading it can tell me exactly what is missing. What do you guys think?

No polio needed for AOS. Your plan sounds good.

You can always upload an image of her DS-3025 and people here can take a look at it.
