I am currently filling out the form, and my fiance has two middle names. Should I put both down when it asks for the middle name?

Put down exactly what is on his passport/travel document. So if both middle names are in his passport, put them down. But if it's not on his passport, then leave it out.

Agreed with those who advised to key-in the full legal name/s as what appears on the birth certificate and passport, may it be first name, middle name or last names.

This is actually not unusual. If it doesn't fit on the item field, you will need to add a supplement to that form. My then-fiance (and now-husband) actually made fun of my name while filling-out the forms and said that US don't accept long names, because I have 2 first names and 2 last names, and so many letters too!
