Not all forms go in AOS package at once?

I came across the April 2014 fillers and I noticed some of the timelines only mention some forms sent out in AOS package but not others , that in my understanding -unless I am wrong-, are part of the AOS package.

Can someone please verify all the forms that adjusting from F1 overstay via marriage I need to send in my package?

This is what I see in some timelines from people in my same situation :

AOS from F1:
04/02/14 - day 00 - 130 / I-485 / I-765 / I-131 sent via USPS
04/04/14 - day 02 - Arrival of AOS packet according to USPS tracking
04/13/14 - day 11 - Email/text receipt notification
04/14/14 - day 11 - Checks cashed
04/25/14 - day 22 - Biometrics appointment letter received (dated 04/16/14) for 05/13/14

And these are the forms I am working on to submit in my AOS package :

I-130 Petition for Alien Relative

I-485 Application to adjust status

I-765 Application for EA

G-325A Biographic Information of both spouses

I-864 Affidavit of support

*I am not sending I-131

And I also need help understanding when to submit I-693 Report for Medical Examination and Vaccination Record. Is this form submitted after Biometrics? do I attach a copy of my vaccination records? if I am missing some vaccination, can I get them at my Dr's office? I am very confused on how to work with this form and when to submit it.

You send all the forms at the same time. You need to do a medical exam (I-693) before you send in your package, with a USCIS-designated Civil Surgeon. You can find a Civil Surgeon at uscis.gov. They will put the medical results in a sealed envelope and you send the sealed envelope in with your initial package.

I wouldn't worry about what people put in their timelines, it is just probably shorthand for the main forms that give a benefit. No one is excited to mail the I-864, you know?

So I take this form I-693 at the time of the medical examination for the Civil Surgeon to sing it? so I can bring my vaccinations records to the medical examination and they will tell me if I am missing any vaccines that are required, they would probably give them to me right there...is that correct?

The medical is usually in 2 appointments (you need time for the blood and TB results). At the first appointment, you will learn what shots you need. You can either get them from the Civil Surgeon (convenient) or go elsewhere and get them, and bring proof of vaccination back to the Civil Surgeon at your second appointment.

The Civil Surgeon appointment is not covered by insurance, so a lot of people prefer to go to their normal doctor to get the shots, or a county health clinic, or something, to save money.

Don't try to figure out what shots you need on your own. It's just a waste of time. Do this all in consultation with a Civil Surgeon, who will consider your age, your country of origin, your vaccination history, your medical history, and everything to determine if you need any additional shots. Make an appointment today, and talk it over with the Civil Surgeon.

The medical isn't only about shots, it is also a physical exam, TB and syphilis tests, stuff like that.

Your normal doctor won't know what shots you need either.

Don't be afraid, use the Civil-Surgeon locator at uscis.gov, call around and ask for a quote for the immigration medical, and proceed. Try to find a quote around $200.

Do your medical, get the results in a sealed envelope. Easy peasy.

One more thing, should I wait until I fully recover from this kidney infection and have no signs of it? urine test at Dr office yesterday showed negative and blood count was normal, but for a strange reason I am still running mild fever (99.8) and feel ill, like mild flue symptoms and it's been 3 days now. Not even over the counter meds give me relief. My Dr said it might be a virus , he could not help me much about why I still feel ill when infection is almost gone. Today is last day of antibiotics and back doesn't hurt anymore. Other than I don't have any health issues.

Call them first and ask for quotes. The price is variable, and you don't want to be charged $500 when you can get it for $200.

Yes that is a good tip, I was also advised by one of the immigration attorneys we consulted here in town that we should pay in cash

Done! I have appointment with Civil Surgeon for first stage of medical exam, coming back on Friday to check my arm for TB reaction, and then next week they will call me to sign the forms, received my copies and sealed envelope. $235

I might be right on this, as if you have had a physical within 1 year, then a cival surgeon is not necessary, to file AOS, just forward your vaccinations record from your medical abroad, my wife went to "NO" cival surgeon, and has her 2 year GC, no problem! (EAD/AP also)

To me this cival surgeon thing is "way over-done" when not necessary!

Just my opinion.,.,...
