DS - 160 questions

My fiancée will get her Mexican ID (credencial) and passport this week. I already received the NVC letter and spoke to an agent stating my package is in Juarez. I will visit my fiancée next week to fill out the DS 160. I plan on doing as much here in southern California and having her finish (under my supervision) in TJ.

I ran into a sample DS 160, and have some questions:

1) Where do I get the National Identification Number from?

Under travel information:

2) Intended date of arrival? Do I make one up, unknown when I have interview to approximate.

3) Have ever you ever been in the usa before? yes, but she does not remember , she was a kid.

4) have you ever been issued a visa? Yes but does not remember details that was over 14 years ago and she was a kid.

Under Family information:

5) She knows her father is in Fresno California. But she does not know the address.

it asks for infor, Put Unknown?

6) Do you have documentation to establish that you have received vaccinations in accordance with U.S. Laws????? I am sure she has been vaccinated in mexico but unknown if they are in accordance with U.S. laws.

3) Have ever you ever been in the usa before? yes, but she does not remember , she was a kid. The answer is still Yes.

4) have you ever been issued a visa? Yes but does not remember details that was over 14 years ago and she was a kid. The answer is still Yes.

it asks for infor, Put Unknown? I am not sure you can enter Unknown on an electronically submitted form. 6) Do you have documentation to establish that you have received vaccinations in accordance with U.S. Laws????? I am sure she has been vaccinated in mexico but unknown if they are in accordance with U.S. laws. If she does not have vaccination documentation, then check No. If it asks for an explanation, put that she is applying for a K-1 and will receive them for adjustment of status in the US. Vaccinations are not a requirement for entry with a K-1. If she has been vaccinated and has vaccination records, then you could also just check Yes She will be getting any that are required before applying for AOS.
