Ok, I looked at the example form, but it doesnt show..

where is says "full name and address of employer"

does it mean

like the establishment name and city, state..

or does it mean establishment name, ADDRESS, city state?

and if it makes a difference it is jamaica..so they dont really have addresses anyway.....

I took it to mean if someone wanted to send mail to my fiance's employer - that is the address that should be provided there. ' How that translates to Jamaica, I have no idea. They 'don't really have addresses'?
