G-325A Employment History Question!

My fiance is currently filling out his G-325A forms and is having some confusion on how to correctly fill out this portion.

He has worked the same job for the last 5 years but in between, every summer, he was able to have off from his job in Scotland and work at a summer camp in America through a company called "Camp America".

I'm not sure if this will make sense, but should he be writing multiple lines for each job, for example -

KFC 09/2013 to Present

Camp America 06/2013 to 8/2013

KFC 09/2012 to 06/2013

etc.

and so on for each summer he worked. Or should he just use one line to fill out the whole complete time he worked at KFC and then underneath all the times he worked at Camp America, for example -

KFC 10/2009 to Present

Camp America 06/2013 to 8/2013

Camp America 06/2012 to 8/2012

Camp America 06/2011 to 8/2011

etc.

Thanks for any help, hope I made some sense!
