So I was looking over the guide for the AOS steps after marriage. I'm aware of all the steps but have a couple questions did read some older threads around 2010-2011 so want to re ask see if anything has changed or been updated.

1st I would like to know whats the current address to send the AOS packet I keep seeing its Chicago. Can anyone supply the Chicago address or addresses.

Next, regarding translate I am going to translate my wife's birth certificate its in spanish but how exactly do I go about that? On another page write word for word or get a copy and write over the translated word. How exactly are they wanting a birth certificate translated?

Last, She did her medical exam in Ciudad Juarez BUT it was only the medical exam since I read on her all was needed that vaccines werent going to be needed. Well that packet was given to the embassy there and the packet we got with the visa was given at the border when crossing thru land so were only given the I-94 stapled to the passport and i believe another paper. So my question is regarding the medical exam is she suppose to do another medical exam here with a civil suergon again this time to included vaccines? She does have her shot records or the booklet they provided in Mexico I guess to cross check what she will need over here. I am wondering this to see if we will in deed pay for a civil suergon and vaccines cost? Because I noticed there is a I-693 form that is regarding the medical exam. So I want to no what is the exact process to have a correct I-693 done thats my biggest fear in messing that form up.

When my husband translated his birth certificate, he used a Word doc and just tried to make the layout as close as possible to the original birth certificate, Where there is a seal, just put "Seal" in that area. Put this certification on the English translation and include it along with a copy of the original that is in Spanish > http://www.visajourney.com/content/translations

Juarez tends to skip vaccinations for a K-1 applicant. Take her shot records to a designated civil surgeon for any missing vaccinations and to have them transcribed onto an I-693. You do not need a new medical(see instructions for I-693 and I-485), but you may need to call around a bit until you find a civil surgeon willing to do just the vaccinations without insisting a new medical. So I called a couple places but they said she would need to do a new medical b/c they cant put on the i-693 that she had a medical with them if she only gets the vaccines. So do I just keep calling other areas? What happens to the medical section is that just left off blank? Do I send the i-693 with only the vaccine part complete?? We don't have a copy of the medical record done in Juarez back in Jan/2014 reason being b.c was sealed and handle over the embassy during interview had to be sealed.

Another thing we she only has vaccine records of around 15 years and up. Would that be an issue or does she also need a copy of her childhood vaccine records to show the civil surgone office?

Yes, keep calling around. You may have to call some a bit farther away from where you live to find one that knows how to deal with a K-1. The all should already know how, but since they can charge what they please, they try and get more money by insisted you need a new medical when clearly the instructions state you do not. Not many people get a copy of their medical. Only a very few countries provide that, and they are usually in an extra sealed envelope they provide for AOS.

They need to make sure you have all the vaccinations required for AOS, which is at least one in a series of MMR, Tdap/TD, varicella(or history of chickenpox) and flu shot(only during flu season- which is it not anymore at the moment). Page 1 and the vaccinations page needs to be filled out when adjusting from a K-1. The other pages will remain blank. They should put the original I-693 they fill out in a sealed envelope for you to include with your AOS paperwork, and give you a copy before sealing it.

Just take whatever vaccination records she has with her to the appointment with the civil surgeon. They should be able to tell you what she needs for AOS. If they are in Spanish, let the civil surgeon know and ask if you need them translated or in Spanish is okay. The civil surgeon we went to for my husband let us bring the records as they were, in Spanish, and he was able to know what the vaccinations were listed on them.

If you keep calling around and cannot find anyone to do just the vaccinations, then you might need to just go to the one that offers the cheapest medical and bite the bullet and get it done. Be sure to tell them when you call that the I-693 is for a K-1 adjustment of status and a K-1 is not required to get a new medical if their overseas medical was done within a year of filing for AOS, and that it is stated right there in the instructions for the I-693 form. > http://www.uscis.gov/sites/default/files/files/form/i-693instr.pdf

Q57. Many applicants come to a civil surgeon requesting only the vaccination portion of the medical exam. Is there a way for the civil surgeon to determine whether the applicant falls within one of the groups that are exempt from the full medical exam (e.g. refugee, asylee, K or V nonimmigrant visa holder within one year of the overseas exam with no Class A conditions). Is the applicant required to show the civil surgeon Form I-94 or other document showing his or her immigration status?
A57. No. Generally, applicants are not required to verify their immigration status to the civil surgeon. The applicant is only required to verify his or her identity by providing a government-issued photo identification. USCIS will confirm whether the applicant’s immigration status and previous immigration medical examinations overseas only require the applicant to complete the vaccination portion of the medical exam in the U.S. Therefore, if the applicant believes he or she only needs a vaccination assessment, the civil surgeon may perform just the vaccination portion of the medical exam and complete pages 1 and 5 of Form I-693. If USCIS determines that the applicant requires the full medical exam, then USCIS will return Form I-693 to the applicant with the appropriate instructions.

Thanks for all the tips guys! Found a place saturday after calling went in b4 they closed at 1pm was out in 30mins. Thankfully she had those 3 shots recently taken last year and only need to transfer the vaccines info onto the form I-693 $35 bucks later done. Nothing else needed sealed envolope and copy of it as well.

Now need to get the I-485 ready for her along with this and send out to get that GC for her and EAD/AP, hope everything else goes smooth from whats left for GC!

One final question we got married thru the civil court and I haven't looked at the marriage certificate if she took my last name or not. I just remember both filing out our names as on our birth certificates. So was wondering is my last name automatically transferred over to her along with her last name and if so. Is that how I fill out her name on ALL the NEW DOCS? My last name followed by her father's last name only. Or is it filled with the name used for the K1 Visa?
