Hi,I'm a K1 visa and I had my medical examination October of last year... I already passed my I-485 documents together with my vaccination supplement, the one I received from St. Luke.. Now, while browsing I found out that I have to pass a I-693 page 1? Is it true? cuz, I read the instruction of form-485 and it was not stated there that for the K1 they should fill out the page 1 of form I-693.It was just stated there that if I had a medical examination within past year, the only thing I will submit is my vaccination supplement. Please help me, I'm afraid that they will return my documents cause when I track it, it was delivered already.What will I do?

Hopefully, Hank will reply to your post. Some have been successful with the "vaccination supplement" and some have not been successful. "Some" have received an RFE for the I-693 package. It might be in your best interest to be at least prepared to submit the complete I-693 package. It may or may not be needed.

No you do not need to send the I-693 in most cases, that is only required with the K-1 if there are special medical conditions. You only need to send a copy of the DS-3025 vaccination record with the I-485... you are fine. Read page 4, item 7c of the instructions for clarification on this.

As a side note, St. Lukes generally administers the required vaccinations as part of their medical. If so, it isn't necessary to have a civil surgeon fill out an I-693. You simply submit your DS-3025 with your I-485 packet.

I had the vaccination supplement (first page I-693) certified by one of the board doctors. It costed something like $35. I think this is the accepted method to not get an RFE. Otherwise I think It's a gamble. It seemed like it was worth it to do it.
