I believe that when my new wife got her vaccinations in her native Japan (before being granted a K-1 Visa), they missed 1 or 2 vaccinations. The City Clinic here in New York will give it to her for really cheap, but they need some sort of paper stating that she needs a measles shot and for what purpose, she has this, but it is in Japanese text. Of course she needs to get it translated, but how official of a translation does it have to be. Obviously, they won't believe it if she translates it herself. Does she need a doctor or lawyer to translate and sign?

I just called the Japanese Consulate here in NYC and they told me that vaccination/medical record translations is something that they don't offer. According to the checklist on the VJ website, I should go to a Civil Surgeon, I'm going to search for one, but then what about having it translated for him beforehand. I guess I should ask that Civil Surgeon, what trusted source he'll accept.
