I-485 (2 quick specific questions)

I was looking at the example form presented on this website because I will be helping my new wife fill it out.

1.) On part 2, letter C, it says to attach a copy of the fiance petition approval notice. That sounds like something we should have, but what exactly does it look like? I have so many papers from all of this process.

2.) On part 3, it asks for her "non-immigrant Visa number". Where can I find that?

A little background info.

-We don't have a SSN because we forgot to get it.

-Our "Admit until" date on the I-94 is April 25, 2014

-We got married on April 16, 2014

-She is a Japanese citizen and I am a US citizen

Approval notice is the NOA2 from the I-129F. It says that the I-129F petition has been approved, with "Notice of Action" on the top.

Visa number is the red number on her visa.
