What's with all of these medical forms people are talking about? My fiancee is having her medical exam at St. Lukes tomorrow. I thought the only form she needed is form DS-3025 for AOS. What's with forms: 693, 693A, 2053, etc? Does she need any of these forms also?

Rob she will fill-up the forms at St. Luke's when she gets there. The forms are called Personal Data Sheet.

She will need to know the following for the forms:

St. Luke's - Information for forms (Patient Data Sheet forms):

· Name (Last, First, Middle):

· Civil Status: (Single for K1 or Married for K3)

· Case Number: MNL2006XXXXXX

· Classification: (K1 or K3)

· Consul: Manila, Philippines

· Present country of Residence: Philippines

· Previous country of Residence: N/A

· Passport Number:

· Place where passport was issued:

· Date when passport was released:

· Age:

· Birthdate: MM/DD/YYYY

· Birthplace:

· Date of previous Chest X-Ray:

· Is this your first time for a medical here at SLEC?:

· Intended date of departure for the USA: MM/DD/YYYY

· Intended Port of Entry (POE):

· Complete address in the USA:

· Tel. No. in the USA:

· Is your petitioner still alive? Yes

Use fiance/husband info for these:

St. Luke's - Questions during the exam:

· Have you had Chicken Pox and do you have any scars from that?

St. Luke's - Notes:

· Keep your Official Receipt handy, since this will be asked from you every now and then.

· It's good idea to bring a jacket as it is sometimes cool inside SLEC.

· You can bring bottled water inside but no food.

· If necklace is worn it must be removed for X-Ray.

· Hair will need to be tied back for X-Ray.

· Normally the medical exam process takes two days. Day 1 is the medical exam and Day 2 is the vaccination. Some may complete both in one day.
