Need Civil Surgeon in Baltimore Area

Hello all,

We are looking for a civil surgeon in Maryland, preferably around Baltimore, to conduct a vaccination supplement. We've called around and are getting quote for over $300 just for filling out the supplement (not a full medical). My husband will also need the varicella vaccine (even though he had chicken pox as a kid!).

Frustrating as we had our interview in September, where we were told we were approved, and only found out we needed the supplement today. Been waiting by the mailbox everyday in vain!

All I can say is please dont go to the one on RT40 in Essex. They had very bad service and not very helpful at all. Yes they overcharged my husband also and they were really out of the way.

Do you have the vaccination worksheet (DS-3025) from the medical in Kosovo? You can get a civil surgeon to transcribe it to the I-693A.

I read somewhere in the forums that there's a civil surgeon in Texas you can send the DS-3025 to, and they'll transcribe it for $10... sorry, didn't bookmark that thread... anyone read about this as well?

I don't think he is cheap but his service is first rate.

I wonder where you here in MD.? My husband just called today to just transfer my vaccination records only they told us they will charge $30 luckily we found this civil surgeon:
