questions about the medical vaccination form

Hello vjers! We're still reviewing some stuffs about this aos.

It's been 4 months since my husband got his medical before he entered here in US. They gave him a CD

and a vaccination sheet.

And I just read that that medical is good for 1 year. so it means that he don't need to go on another medical but do we need to send the vaccination sheet from his country and the Cd. and do we need to send any form about medical??? it's very confusing. thanks in advance!!

With AOS from K1, you are not required to have another medical examination as long as your I-485 is filed within one year of your (overseas) medical examination. You are still required to complete the I-693 and to do this you will need to find a Civil Surgeon (see civil surgeons list: http://www.visajourney.com/content/us-civil-surgeons) have him complete the form, certify it and seal it in its own envelope. Include this in your AOS packet and you are pretty much set.

The CD is for you. It has the xray on it. The DS-3205 sheet is what you send with your I-485.

If the DS-3205 was properly filled out and no vaccinations were missing, and you file I-485 within a year of the medical, then you do not need an I-693.

It is not still a requirement to get an I-693, if you had all the required vaccinations marked off on a properly filled out DS-3025 at the time of your K-1 medical.

3. What if I am a K nonimmigrant visa holder and already had a medical exam overseas?

If you were admitted as a:

a. K-1 fiancé(e) or a K-2 child of a K-1 fiancé(e); or as a
b. K-3 spouse of a U.S. citizen or a K-4 child of a K-3 spouse of a U.S. citizen; and
c. You received a medical examination prior to admission, then:

(1) You are not required to have another medical examination as long as your Form I-485 is filed within one year of
your overseas medical examination, and the medical examination did not reveal a Class A medical condition, or
if you did have a Class A medical condition, you received a waiver of inadmissibility and have complied with
the terms and conditions on the waiver.

(2) Even if a new medical examination is not required, you must still show proof that you complied with the
vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part
of the original overseas medical examination repor
t, you will have to have the vaccination report completed by
a designated civil surgeon. In this case, you are required to submit the vaccination record and page 1 of Form
I-693.

so that ds-3025 is the vaccination form?

and if it's not properly filled out, I need to get a I-693 and go to a civil surgeon?

Yes. if you did not have all the required vaccinations marked off on a properly filled out DS-3025, then you will need to get any missing vaccinations and have a civil surgeon transcribe them all onto an I-693. Finding one that will not insist on a whole new medical can be a challenge sometimes though.

The AOS can be so confusing! My medical was done back in october, 2013 and I read that I didn't need to get the medical all over again. So, in February, I went to a civil surgeon (who qualified and was on the list as recognized by USCIS) and had him certify pg. 5 of the i-693 showing that all my vaccines were up to date. I only sent in Pg1 and 5 of the form i-693 (which the Dr. Signed and SEALED in an envelope) as it says right on page 5 that you need only submit those two pgs if the vaccination assessment is all that is needed. I paid the Dr. $65 to sign off. Hope this helps!

I didn't even send in an I-693 with our K-1 AOS package because we didn't think to make a copy of the DS-3205 and dont even have a copy of it. I guess we will see what happens.

I am in the early stages of this process right now. My wife did the medical exam in August or October of 2013. She didn't get the measles shot and thus needs to get that done in order for the I-693 to be properly completed. The thing is right now we just have a Japanese copy of her records. We will need a translated copy of her records in order to communicate to the new Civil Surgeon that all she needs is the measles shot and nothing else. There was a translated copy handed to the USCIS before she entered the country (it was in a sealed envelope though). Shouldn't this mean that the USCIS already has a translated copy that shows all she is missing is the measles shot?

My fiance arrived from the Philippines a little over three weeks ago. This vaccine stuff is confusing. When she had her shots she was told she needed another one after one month. And attached to her DS-3025 was a small note that said "Further vaccinations are recommended as required". But I was reading this post and I'm not sure if she actually needs more shots. Below is a copy of her DS-3025. Could anyone clarify for me if she needs more shots or is she good to go? Thanks.
