Q: AOS K1 - I-693 / DS-3025

Hello everyone,


I petitioned my fiancee (now my wife) thru K1 visa. She got her Social security number now and we got married recently. Now, we’re ready to adjust her status.


Based on my research, I will have to send I-485(AOS) and I-765(EAD) together with supporting documents.


She had her medical prior getting her K1 visa as a requirement. Here’s her DS-3025 looks like.

Now, I’m confused on I-693. It says

I also heard that AOS only need MMR Tdap or Td or DTP (within last 10 years) Varicella (or VH if had chickenpox disease). Flu shot only if exam during flu season. If she had all the required shots at the time of the medical or if the form says incomplete but have a blanket waiver the ones needed for AOS., then we don t need to see civil surgeon. Is that correct?


Can someone please evaluate her DS-3025 if she already have the required vaccines and we dont need to see Civil Surgeon? or can someone please advice us on what is the next step regarding I-693/DS-3025?

Yes you are fine. You only need the first shot in a series for AOS.

My now wife received the DS-3025 but in her case it was only signed and had nothing checked or marked, is that normal? She was told that due to the type of visa K-1 she didn't need any shots, that plus I think she did get some vaccinations before so I'm not sure what to do (or think).

The clinic gave her a cd (apparently blank when she checked on the computer). Any ideas?

Depending on what country, it can be normal. For instance, Juarez does not tend to give vaccinations for K-1 applicants and provide no DS-3025 or a blank one. You will need to see a civil surgeon for any missing vaccinations and to have them transcribed onto an I-693.

Hi, I am on the same situation. Find a doctor who can help me completed I 693. I have had already my medical examination at overseas.

My MMR is not checked but i have been vaccinated Measles.

I have got Tdap, and Influenza this January!

Am I okay with this Vaccination Documentation sheet? Or Do I have to attempt again testing?

The DS-3025 was properly completed. Send in a copy of it with the AOS application.

Can anyone confirm that my worksheet is complete and my vaccinations list is sufficient enough to send in for AOS from K1, without the need to see civil surgeon in the US? Its attached in this post.

It seems like your vaccination sheet is completed. Yours like mine and I had asked and showed to my friend and she told me only sent this and she did not get RFE for that.

My fiancee finished her medical and vaccine requirements with a civil surgeon in Buenos Aires for her K-1 visa. However, the civil surgeon filled out all of her vaccines on page 5 of the I-693, and not on a DS-3025.

Is it permissible/equivalent to submit page 5 of the I-693 with the application in lieu of the DS-3025? If so, does she also need to submit a copy of page 1 of the I-693 with the civil surgeon's signature (even though all of this is in the sealed envelope she will be presenting at the POE)?

As she is still in Buenos Aires, it would be possible for her to go and get a DS-3025 from the civil surgeon if the I-693 page 5 is insufficient.

Interesting. So she doesn't have her DS-3025 after medical, does she? I don't know about K-1 visa process for Argentina, but for Indonesians, you have to go to panel physician that the US embassy put on the list to get your medical done, not just a regular civil surgeon. Most of times, K-1 visa applicants will get DS-3025 form and health certificate like the ones I have now.

Oh okay, the vaccination sheet is very important and since she's already have one as a copy of page 5 of I-693, she should be fine then as long as her medical was done within one year.
