My fiance arrived last month on a k1 visa and we just got married. While reading the guidelines it says that we need to give a copy of DS-3025 Vaccination Record. My fiance told me that he does not have a copy of this. What to do?

December 18, 2023:  Filed I-90 to renew Green Card

December 21, 2023:  Production of new Green Card ordered - will be seeing USCIS again every 10 years for renewal

I really just depends on who you believe here at VJ. The USCIS instructions read as follows: "

If you are a K-1 fiancé(e) or K-2
dependent who had a medical examination within the
past year as required for the nonimmigrant fiancé(e)
visa, you only need to submit a vaccination supplement,
not the entire medical report. You may include the
vaccination supplement with your Form I-485."
Some have been successful following this procedure and some have not been successful.
To avoid any possible delays, and also not knowing the above instructions, I had my gal file her I-693 and was successful. Whichever you choose to do, it would be the least expensive to have any vaccinations/immunizations performed at the local county health department. This is where my gal had hers done and all she needed was the MMR shot and something else - all less than $200 and the local CS did her paperwork for free. I would not expect you to get the CS to do it for free but less expensive than having any vaccinations/immunizations done by the CS.

Not sure why he doesn't have the DS-3025. This is the piece of paper handed by SLEC, together with the CD, after the medical.

If you guys still can't find it, you need to go see a civil surgeon and do the full medical to get a completed I-693.

He received the DS-3025 with the CD from St. Luke's. Not sure what to do if he lost it as you need proof of vaccinations.

They do not need a new full medical. They already had the medical and the documents from it are included in the sealed envelope turned in at their POE. If the medical was done within a year of filing for AOS, then a new medical is not required. It has nothing to do with whether or not they have a copy of their vaccinations.

OP > Are you sure he does not have it? If not, is he sure he had all the required vaccinations? He can put a note in with his AOS paperwork that he had all the required vaccinations and the DS-3025 was included with the rest of his medical forms in the case file he turned over at his POE.
