Medical Examination Done In London UK Results Question

Hello Fellow VJ's,

From my understanding we don't have to fill out a I-693 since my now Husband's (Fiance) examination is still valid for one year.

For the AOS I know we have to send the Vaccination Documentation Worksheet DS-3025 do we send The Knightsbridge Doctors Visa Medicals Certificate of Immunization as well. He also received a cd is this for him to keep or send? Do we send copies of these documents or the originals?

Also on his DS-3025

2. Results

"Vaccine History Incomplete" was checked off as well as the box that says under it "Applicant may be eligible for blanket wavier(s) because vaccination(s) not medically appropriate (as included above).

Not really sure what this means...

Does this mean he will have to do the I-693 here or it's okay?

Its fine, they all say that (from Knightsbridge Medical in London at least). Use the copy of his DS-3025 for AOS as long as you submit it within one year of it being issued. (What you have is a copy or what went into the big white sealed envelope surrended upon entry. Keep another copy for yourself just in case, a new doctor, an employer or school may want to see it)

'The Knightsbridge Doctors Visa Medicals Certificate of Immunization' - what is this - I never got that?

The CD, you can use as a coaster or something, its just an expensive souvenir from the process that you never need again. (Its supposed to have the chest x-ray on it I believe)
