According to the state department the following are health related reasons for denying a visa

  1. Health-Related Grounds:
    Among those persons excludable for health reasons are

  1. Aliens afflicted with a communicable disease of public health significance, including HIV infection;
  2. Aliens who are determined to have a physical or mental disorder and behaviour associated with the disorder that may pose, or has posed, a threat to the property, safety, or welfare of the alien or others, as well as those aliens who have had such a disorder and associated behaviour which behaviour is likely to recur or lead to other harmful behaviour; and
  3. Drug abusers and addicts.

What exactly do they check for when going thru the medical exam? What would be a disqualifier?

Drugs would definitely be a disqualifier. And life threatening or debilitating diseases that make one likely to become a 'public charge' because they cannot work (like MS). They check basic health, heart, lungs, and run a blood test. I believe they do AIDS testing with it along with analyzing for other things, but am not sure.

Timeline

They check for communicable diseases such as HIV, Syphillis, Hepatitis, TB etc. They perform a routine medical exam , blood tests, vaccinations, and a chest x-ray to check for TB. It is all very simple. Nothing more than an annual physical with a chest x-ray.
