Hi Folks,

I grew up in a West African country and I am sure I did get some vaccinations if not all. So here's my dilemma, both my parents have passed and I cannot find any record of any and all vaccination I must have taken as a child.

What do i need to do in this situation in preparing for my medicals?

I would appreciate any and all information.

You can get your blood tested for immunity if you don't want to get the vaccinations again. I was able to get it done for free through my family doctor but I don't know how health care works in your country.

I got an AOS checklist on domicile. In my original AOS package I included a signed cover letter on my intent to move back to the US along with my US credit card statements, US bank accounts, US phone/cable bill, drivers license and voter registration. I'm not sure what else I can include that proves my intent to move?

Try getting a lease or a rental agreement wherever you're staying when you move back. The only other thing would be proof of job offer or applying for jobs.

I have a question of I864. There is some fields that don't allow to input by the machine like

Part 1

- 2d (State), 2e (Zip code), 2f (Postal code)

- 5: A# (my mom does not has it).

Part 6:

3b: Date of Retirement

Will I leave it as blank or put N/A by hand because the machine does not allow to enter N/A for these fields.

Thank you very much for your helps.

2d-f leave blank (input by hand if necessary)

type --none-- for 5a if your mom doesn't have a surname

3b - N/A

A#: Alliance number, not the surname

3b: Because the computer does not allow to input N/A to Date field. Can I put N/A by hand and bring to the photocopy this page, because I don't want some fields are computer and some fields by hand.

Thank you :)

There is no alliance number. That is Alien Registration Number. so "none"

yes. You can hand write any part you need to after printing.

don't try to photoshop anything in though. Just write it by hand.

I leave in Canada now. So i guess i can talk to my family doctor then......

yep.. I said to mine, "Hey I have to show I have immunity for these vaccinations... Can you test for that?"

He said; "Sure!" And then wrote up a lab report requesting said immunity tests.

So I went a got some blood drawn and a month or so later, poof, results!

He then said, "So do you want a prescription for the ones that don't show up?"

I replied, "Sure that'd be awesome"

And off I went to the health clinic and proceeded to get my vaccinations for free that I was missing.

FYI - MMR hurts!

BTW I didn't bother with the Tdap testing, just the MMR and varicella (even though they take your word for it that you've had it.) Just get the Tdap again anyhow, if you don't have a record of it on file. Needs to be updated every 5-10 years anyhow.

Haha nice work! I called them back and they gave me the invoice number, as well. They said they'll send an invoice e-mail in the next 5 days. Not sure if I'll be able to pay the fee before I get the e-mail, but I'll try. :)

You know what? Neither my attorney, nor I, received ANY emails with an invoice number. My poor attorney was very confused that I was way ahead of her. I had assumed that the attorney would get an email as soon as the invoices were ready to pay. NOT!

And I agree - we here on VisaJourney should open an office to help people fill out forms!
