Almost ready to file for AOS

Hi, I'm new here and apologize if I'm asking questions that have been asked a million times before.

I'm almost ready to send in my AOS paperwork (thanks to the incredibly helpful posts here on VisaJourney!) but I have a few questions first that I hope some of you more experienced folks will be able to help me with.

1. Do I need a Social Security Number? I'm a stay at home mom and don't plan on getting a job anytime soon so the only reason I'd need it is if it helps with my application or name change.
2. How do I legally change my last name to my husband's name? Our marriage license only says Bride's name before marriage and nothing about whether or not I would be taking my husbands name. I really don't want to send in all my paperwork in a name that's not legally mine..
3. I had my medical exam in Iceland this January so I'm well within the one year window, now do I just skip the I-693 or do I send in a letter saying I had my exam less than a year ago or what do I do?

4. Anyone have an example of a cover letter by any chance?

1, Do not need SSN to file for AOS, but other places may ask for SSN, such as for bank accounts, insurance etc. It is not used just for work. On that note, even if you have no plans to work or travel, you should still file for the EAD and AP with your I-485. It will give you a government ID while waiting for the green card approval, and you will be able to get your SSN with it, should you decide to go ahead and get it. If you are going to live in the US from now on, you should get one. It will also allow you to travel and re-enter should the need arise while waiting.

2. The marriage certificate acts as a legal name change document. Fill out all the AOS forms in your married name The EAD/AP and green card will come in the family/surname you use on the forms.

Thank you so much for your help, I don't need a SSN to be added to my husbands bank account or insurance but will add those forms for sure if that means I'll have the option of getting a SSN while waiting for my green card! never know what might change while waiting. I actually thought I needed specific dates and travel plans to apply for AP but I'll look into it more.

You don't. You can leave the date and length blank and hand-write Unknown after printing out the form.
