Timeline

I received an interview notice, which has a list of items that are required to bring to the interview. One of them is A completed medical examination (I-693) and vaccination supplement in a sealed envelope (unless already submitted).

I already submitted my I-693 in a sealed envelope when I sent in my I-485 application. However, I do not believe the civic surgeon submit my vaccination records. So, how do I prepare my vaccination supplement? Is there a form to fill out? My immunization record is not in English. Can I just translate it into English and have it notarized? or does it required a civic surgeon's signature as well?

If the civil surgeon gave you a sealed envelope, then it's all there - your HIV/TB test results and your vaccination history. You're all set.

Did the civil surgeon give you a copy of the medical? Your vaccinations are on the second page of the form.

He didn't give me a copy because he charged additional $50 for a copy. Anyway, I don't think he provided any vaccination history because he even didn't ask me what types of vacine I've got. So.....what should I do now?

He didn't ask??? Are you sure it was a USCIS approved doctor??

That's weird. I suggest that you contact this doctor and find out what happened. Technically, if he gave you a sealed envelope, you should be all set - your vaccination history is there. But if he didn't ask, what in the world could he have written there? :wacko:

I suppose he didn't give you any shots on the day of your exam either?

Timeline

The panel physician who did your medical should have given you a copy of the DS3025,

which is the vaccination worksheet. You take it to a civil surgeon here who transfers

it to a I693A that is the supplemental you need for the AOS.

If the civil surgeon did not give you any additional vaccs than you should be set and

everything should be in the sealed envelope.

I do not know about the translation issue.

See the * unless previously submitted * part......so if it was submitted then you don't need to send it again.

Double check with the civil surgeon if in doubt.

Vaccination records should be translated and notorized and taken to a Civic Surgeon who should have filled in form..

I'm adjusting my status from H1B. I was never asked to provide vaccinaion information. I did my physical exam at a hospital and gave the report to the civic surgeon. He saw the report and gave me a HIV test and gave me back a sealed envelope. That was it. He didn't ask me any question in regard to my vaccine. Does that mean I have to the vaccination worksheet again?
