Is it needed to put a Title/Header for supporting documents attachment?

Good day!

Just wanna ask you guys about the supporting documents like the photo copy of passport stamps,

boarding pass, receipts, etc...

And about the photos together in the procedure stated that put them all in one plastic and attach to the paper.

My question is that, Do we need to put Title/Header to those attachments?

Do you handwrite the Title after photocopying or type the Title or what?

or just only sign it and put the date after photocopying?

The bond paper that we are going to use to attach the plastic, Is it labeled or no need?

I am just wanna sure if I'm doing the right thing...So I really need your help guys.

Hope you can share your ideas.

March 22: I-129F application sent by my Fiance

March 28: I-129F opened in lockbox informed thru email

April 2: Alien registration number has changed thru uscis website, case status

April 16: I-129F petition approved

April 22: I-129F mail returned to sender due to undeliverable

May 5: Approved I-129F re-mailed to correct address

May 14: Received I-797 (NOA2) thru mail

May 27: Received letter from NVC Manila Case # thru mail

Paid MRV at BPI

May 29: Completed DS-160 online at ceac website

May 31: Received I-129F copy packet from fiance

June 5: Received hard copy notice of eligibility from the US Embassy Manila

June 5: Received appointment confirmation

June 17: Done medical at St Lukes (1.5 days)

June 17: Cancelled schedule of interview due to mutilated passport

July 21: Interview date (Approved)

July 25: Visa status (ceac): Issued!

July 30: Visa status: for delivery

Sept3: Arrival L.A.X

Sept30: Got married!

Nov 5: Mailed AOS, AED, AP to Chicago, IL

On the procedure stated to keep the original documents and just photocopy them for attachment.

So my concern is that when after photocopying them like for example in a legal size bond paper.

Is it necessary to put Title of that photocopy? or just write signature and date it.

I wrote back the photos, I decrived where we were in the photo and date.

And handwrote tittle on top of the copy, for example in the copy of the boarding pass I wrote on top:

First trip on april 22-2013.

Something like that I did with copies,

except with devorce decre, usc birth certificate, and bio page passport, I didnt write nothing is this copies.

There is no issue titling the pages if you want. I know I just included the copies of the boarding passes and passport pages without any titles. I did label the pictures together.
